We typically ask you to arrive 15 minutes before your first appointment to complete some brief paperwork. Please bring your prescription, insurance card, and driver’s license along with a medication list if applicable.

Your therapist will evaluate you and customize a plan of care for you and answer any questions you may have.

We are in network with Medicare, BCBS, BCN, PRIORITY, HAP …etc, we take almost all insurance plans including Workers’ Comp and Auto. Plus, we always verify your individual insurance plan prior to scheduling your initial evaluation as a courtesy. We also offer private pay rates.

We accept MasterCard, Visa, American Express, Discover, HSA or Flex Spending Account.

In most cases, physical therapy is covered by your health insurance. At OMPT our staff will verify your insurance coverage before your first appointment as a courtesy. We also have Private Pay options and customized payment plans if you don’t have insurance.

Although your doctor may refer you to a specific physical therapy clinic, patients do have the choice about which physical therapist to see. Tell your doctor that you prefer OMPT Specialists for effective, result-oriented care.

State of MICHIGAN allows direct Access to your physical therapist, but your insurance may ask for a prescription to be able to use your insurance.

If you choose to do private pay, then you do not need a prescription or referral (some limitations may apply).

Once we get your information, our staff will register you and start the verification process if you are using your insurance. We will schedule you within 24-48 hrs from your call or sometime on the same day (especially if you are a private pay). Your first appointment will be an evaluation, discussing the findings and the care plan.
